• Hospice Nurse Practitioner - Provide recertification visits and symptom management for patients on hospice service. 
• Pulmonary Medicine Nurse Practitioner - Collaborative practice with hospital-based Pulmonologists providing inpatient consultations.
• PRIME Nurse Practitioner - Sole provider for Progressive Illness Management program. Responsible for providing care to patients referred to this consulting service within local skilled nursing facilities. Management of acute and chronic illness as well as providing pain and symptom management for palliative care patients. Medication reconciliation, coordination and transition of care management upon admission and discharge in facility. Advanced care planning and goals of care discussions.
